* A copy of your ID is required, please upload a copy of your passport. (If you do not have a passport, please upload another official ID such as a driver’s license.)

* The Banquet Party is scheduled to be held, however please note that it may be canceled depending on the situation of COVID-19. Please indicate whether you would like to attend the Banquet Party when registering so we can have an idea of the number of participants. Registration and payment for the Banquet Party will be finalized on site.

4. How to register for JSCN64

Please click the link below and follow the instructions on the screen to submit your registration. If you are unable to proceed to the next page after clicking “Register”, you may need to clear your cache. Should this occur, please follow the instructions below:

5. Changing and canceling registration

Once submitted, registration cannot be canceled. We look forward to receiving a large number of registrations.

6. Proof of Participation (Name Card)

You can download your Name Card from “My Page”. You will also be able to confirm and change your registration information after logging in. Please note that you will be unable to download Proof of Participation after 17:00 JST on June 30, 2022 (Thurs.). The Secretariat is unable to re-issue Proof of Participation, so please download it before the date above.

For those attending on site
Please make sure to print your Name Card in color and bring it with you when attending on site. Those without a Name Card will be unable to enter the venue. You can obtain a Name Card holder from the Information Desk in the Lobby on 2F.

7. How to access the streaming site

Please click on the “Streaming Site” button on the JSCN64 website. You can log in with the ID and password sent in the Registration Confirmation email.

Reproduction or recording of any part of the streaming site or its contents is strictly prohibited. Re-posting of screenshots or any downloaded information on other websites is also strictly prohibited. Please do not share your login ID and password with any third parties.

Regarding the handling of personal information
The information submitted upon registration will not be used for purposes other than the management of The 64th Annual Meeting of Child Neurology. We will also take all necessary security measures to ensure that the personal information you provide will be kept confidential.
